Become a Patient
We generally can offer you an appointment within 2 weeks to be seen by a dentist. Call our office at 612-626-3233 and our reception staff can assist you in selecting a provider.
Prepare for Your Visit
Please bring your dental insurance card, and a picture ID, and a list of your current medications and doses prescribed. Also, any forms that were sent to you or that you filled out online, such as a health history and admission forms.
Price and Insurance Information
We are preferred providers for Health Partners and Delta Dental, however we are not affiliated with state-funded programs. Our fees are comparable with private dentists' office fees.
