Refer an Endodontics Patient

Online Referrals

To refer a patient to the Graduate Endodontic Clinic, please follow these steps:

1. Prior to referring a patient to the Graduate Endodontic Clinic, gross caries should be removed and the restorability of the tooth should be determined.
2. Complete the Referral Form and attach x-rays to the form. Make sure all x-rays are dated.
3. You will be notified via email to confirm your form has been submitted.
4. After 72 hours, your patient should call to schedule an appointment at 612-624-2661
