Refer an Endodontics Patient

Online Referrals

To refer a patient to the Graduate Endodontic Clinic, please follow these steps:

1. Prior to referring a patient ot the Graduate Endodontic Clinic, gross caries should be removed and the restorability of the tooth should be determined.
2. Complete the Referral Form and attach x-rays to the form. Make sure all x-rays are dated.
3. You will be notified via email when you submit the form and when the form is processed.
4. You will receive a letter from the treating provider when the referral is closed.
